How to resolve the error "Cannot upload extension .csv file to Switchvox using LDAP"
If you are receiving an error uploading an extension .csv file after enabling LDAP, the issue is most likely that your .csv file headers are not correct.
If you have set Switchvox to LDAP mode, (Setup > Extension Settings), the header-line must include username. Values for username are not required, but it must be in the header. This is meant as a reminder of how important usernames are when in LDAP mode.
The required headers for importing the extension .csv when using LDAP mode are: ext, fname, lname, email, title, location, password, voicemail_password, phone_password, digium_phone_mac_address, username.
