Use the link below to download the required Letter of Authorization (LOA) that must be submitted with all port requests. Please don't forget to submit a copy of your most recent bill, dated within the past 30 days, along with your LOA.
Please ensure your letter of authorization is fully complete, signed, dated, and legible. Electronic signatures are acceptable.
We can accept file formats such as PDF, JPG, PNG, TIFF, and GIF. Please avoid using cell phone photos or copies of low-resolution faxes. Use multiple sheets if necessary. If possible, please combine multiple pages into a single document.
