CG6x6x board fails to start: 'cannot lift reset 121'

Symptom

When trying to bootup a Dialogic CG6x6x Media Board, occasionally the board fails during boot with the "cannot lift reset 121" error, but most of the time the board starts without a problem.

Reason for the issue

The symptom may be caused by the power supply in the chassis not providing enough (or just enough) power for the CG board, so most of time there is no problem, but sometimes it goes over the threshold and the error occurs.

Since the board requires the most power during bootup time, if the power supply on the system doesn't provide enough power then the board may fail to boot.  But if it does get past the boot stage then everything works fine since less power is required during operation.

Fix / Solution

Move the CG6x6x board to another chassis with a more powerful power supply, and see if the problem is still there.  If this resolves the issue, then the power supply on the original chassis should be replaced with a more powerful one.

Workaround

If there are unused cards or boards in the chassis remove them to reduce the load on the power supply.
