The following document is Fonality’s cross-shipping form and policy. Please completely fill-out (all information is required to process your request) the following form and fax to: 310-641-6436
Once we receive your fax, we will process your cross-shipping request within one business day. The only shipping method for cross-shipments is Standard Overnight via UPS.
Confirmation of receipt and processing of your cross-shipping request will be updated in your RMA ticket.

Please note the following
Fonality’s Cross-Shipping / Advanced Replacement program is an advantage for customers. Fonality has taken the time and consideration to provide this program for customers; therefore, the following Terms and Conditions will be enforced:
By authorizing, you agree that the credit card will be charged by Fonality to secure the replacement shipped. Authorized signature must be signed exactly as it appears on the card.
You agree to a $25, non-refundable processing fee.
You agree to pre-pay for a replacement, paying retail, and will be credited back the retail amount only (not the processing or applicable shipping fees).
Warranty exchange products are refurbished replacements.
Customers are entitled to one Advanced Replacement at a time. The original product must be received before another replacement for cross-ship can be issued.
All FORMS must be completely filled out and require a valid signature in order for your request to be approved. Printed names will NOT be accepted as a valid signature.
Replacement products will ship out within 1 business day of RMA approval by Operations Management.
RMA's approved by Operations Management after 2:00 PM PT will be considered approved on the next business day.
Once you’ve received your replacement product, customers must have the damaged product at our facility no later than the end of the 14th day. Any package received after the 14th day will be refused and no refund will be given.
For security purposes, customers will be automatically charged before we ship out the product and will only be given a refund if the product is received within the given time period without physical damage.
Any physical damage found on your product, including shipping damage, will void the warranty. If the product is received with damage, charges will not be removed from your credit card.
Physical damage is NOT covered under your Fonality warranty under ANY circumstances and automatically VOIDS your warranty on your product. Physical damage consists of any physical alterations to the product.
