SWVX-13336 Default Fax Extension not populated when modifying VOIP Provider

Description: When modifying a VOIP Provider under Setup -> Call Routing -> VOIP Providers, the Default Fax extension is not populated automatically. This can cause the value to be cleared if the Provider is subsequently saved and the extension has not been re-entered.
