(12003) Database sql execution failed

Unable to create new extensions and getting the following error: (12003) Database sql execution failed.

  1. From /admin > Setup > Templates modify the 'default' template

  2. Go to the 'Permissions' tab

  3. Save the default template.

Unable to render {include} The included page could not be found.
